Job Description

The position is a combination of conversation therapy and rehabilitation coordinator. In your role, you will work with conversation support, both physical visits and internet treatment. As a rehabilitation coordinator, you will coordinate care for patients who are on sick leave and where care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Agency, or the municipality's social services. Your tasks include supporting doctors and being able to create clear rehabilitation plans together. An important part of the role is to coordinate and collaborate both internally and externally, all to provide support to the patient and help prevent and shorten sick leaves. The position is divided into approximately 60% conversation therapy and the remaining approximately 40% rehabilitation coordinator.

Work Group

We are about thirty-two employees at the health center who enjoy working together in an open work climate. As a conversation therapist with us, you will work closely with the other employees at the health center. The health center has a medical reception, specialist nurse receptions, nursing receptions, child health care, psychosocial competence, foot care, and a laboratory. We are also responsible for several nursing homes. We also have responsibility for palliative care (PAH) at home.

About You

You are a trained psychologist or social worker (or education deemed equivalent by the employer) and have basic psychotherapy training in CBT (step 1). Being a licensed health and medical care curator is an advantage.

Experience in primary care is also an advantage. We prefer that you have experience working in a team and that you are familiar with our journal system, Cosmic. Furthermore, you must have a command of the Swedish language in speech and writing, corresponding to C1 level.

To thrive and fit in with us, your personal qualities are important. We are looking for someone with a high degree of personal maturity. You have good collaboration skills but are also independent in your way of working. You can make professional assessments, start and finish treatments, and have a good ability to structure your work. You are communicative in both speech and writing and are a problem solver both alone and together with others. We place great emphasis on personal suitability.
