The primary care in Västerbergslagen (Ludvika and Smedjebacken municipalities) is looking for a district nurse/nurse for the Mobile Team and Ludvika health center. The position is a shared position of 50/50.

Nurse/District Nurse, Mobile Team/Health Center Ludvika

Job Responsibilities

The mobile team consists of doctors and nurses who make home visits for patients who have difficulty accessing care due to physical, mental, or cognitive difficulties. The mobile team is intended to complement other health and medical care for our most vulnerable patient groups, elderly multi-sick patients, patients with chronic diseases, and patients with complex symptoms, and to increase security for these patients. The assignment includes collaboration with other actors, Ludvika and Smedjebacken municipalities, 1177, palliative team, ambulance, health centers, inpatient care, police, and other health and medical care.

The Ludvika health center is located at Ludvika hospital and has approximately 15,000 registered patients. As a district nurse/nurse, you will work in telephone counseling, district nurse reception, and assessment reception. We have specialist clinics for asthma/COPD, hypertension, dementia, and diabetes. The health center also employs physiotherapists, occupational therapists, doctors, nursing assistants, psychotherapists, psychologists, pharmacists, medical secretaries, and midwives.

• Licensed nurse
• Driving license B

Meritorious:

• Further education to district nurse
• Experience from primary care
• Experience working in the municipality

We would like you to have an interest in your own specialist clinic at the health center. We are looking for someone with good teamwork skills who sees the whole patient's issues, creative problem-solving ability, is unpretentious and flexible. We place great importance on personal suitability.
