The shift towards close-to-home care implies a changed approach. We work at the clinic and in home environments, which gives us extensive knowledge of our patients. We strive for continuity and good relationships. Based on patient needs, we are ready to introduce available technology that can improve and simplify our ways of working. The number of patients registered at the health centre is approximately 14,000, of which a large portion are elderly, leading to growing home healthcare (HSV), currently with approximately 160 connected patients. On the physician level, we are responsible for three special housing facilities (SÄBO) including short-term places, totaling approximately 116 places, as well as specialized palliative home healthcare (SSIH in Trosa and Gnesta) with approximately 20 connected patients.

Your Responsibilities

In collaboration with the municipality's nurses, you are responsible for patients in HSV and SÄBO. Your daily work at the clinic includes patient visits ranging from planned follow-ups of chronic diseases to assessing and handling acute conditions. Here, you become an important team player working closely with other professions. You need to be available by phone to support the nurses in HSV and SÄBO. You are also part of the health centre's dementia team in connection with dementia investigations and collaborate with our heart failure nurses. As part of the role, you supervise ST-doctors during their placement with us.

Your Qualifications

We are primarily looking for you who are a licensed physician and specialist in general practice. However, we are also looking for you who are a licensed specialist physician in geriatrics, palliative care, or internal medicine, with a strong interest in primary care. Good language skills in Swedish and good IT skills are required.

You are flexible, reliable, and responsible, and have the ability to work solution-focused even in stressful situations.
