Do you want to make a real difference in people's daily lives and help shape the future of close care? Älvsbyn Municipality is now looking for an engaged and independent licensed occupational therapist to join our wonderful rehabilitation team within home health care.

About the Position

As an occupational therapist in our municipal primary care and home health services, your main task is to promote independence and quality of life for our patients in their own homes. You will primarily encounter elderly individuals as well as people with physical, mental, or cognitive disabilities.

You work based on the patient's own goals, focusing on health promotion and rehabilitation. You are part of a supportive team with occupational therapists and physiotherapists, but you also collaborate closely with nurses, home care services, assistant nurses, and relatives.

Your main responsibilities:

• Conduct activity, function, and environment assessments in the patient's home environment.
• Trial, prescribe, and follow up on technical assistive devices.
• Plan and initiate rehabilitative and daily life rehabilitation measures.
• Assess the need for and write certificates for housing adaptations.
• Supervise and delegate rehabilitative measures to care staff.

Who are you?

We are looking for someone who is confident in their profession, has a coaching approach, and puts the person's needs at the center. Since the work takes place in the person's home, you must have a good ability to plan and prioritize your workdays independently.

Requirements for the position:

• Licensed occupational therapist.
• Very good knowledge of Swedish, both spoken and written.
• A B-driver's license is required as travel is part of the job.

Meritorious qualifications:

• Experience working in municipal home health care or elderly care.
• Experience with documentation in the Combine medical record system.
