Discharge Coordinator for Orthopaedic Ward, Umeå

The Centre for Musculoskeletal Research and Clinical Research (Rörelseorganens centrum) in Umeå is responsible for Orthopaedics in the northern healthcare region, which covers the four northernmost counties. The Centre is divided into sections: Arthroplastics and Tumours, FACTUM, Trauma, and Spine, all of which perform thousands of patient visits, treatments, and surgical procedures annually.
The Centre for Musculoskeletal Research and Clinical Research in Umeå is a University Hospital Unit (USVE) where research and student supervision are a natural part of clinical practice. We have outpatient clinics, inpatient wards, a day surgery unit, an administrative unit, and an orthopaedic technical centre. Our care is characterized by a holistic approach where many different professional categories are engaged in the patient's care process and collaborate for the best results, with the vision of keeping Norrland on its feet and in motion.
The Orthopaedic Ward has close collaboration with Umeå University through our newly opened clinical education department. Educating students and employees is close to our hearts. Through this new department, we achieve a unique blend of highly specialized care and clinical education. The Position:
We are now looking for a Discharge Coordinator who will participate in the discharge process for Orthopaedic inpatients. There is 1 position available.
Responsibilities:
As a discharge coordinator, you contribute to taking good care of others and helping people feel secure. On the Orthopaedic Ward, you will have the opportunity to learn a broad range of orthopaedic procedures, from planned admissions to emergency care involving trauma care and advanced spine surgery.
At our facility, the discharge coordinator plays a vital and clear role in the care team. You coordinate the planning for the patient before they leave inpatient care. You work closely with the patient, their relatives, the care team, and other healthcare providers. We work in fast-paced care chains where mobilization and return to previous life activities are sought after surgery. As a coordinator, you identify risks, assess needs, and coordinate care to ensure a safe discharge home.
Qualifications:
- You are an engaged colleague who contributes knowledge and joy.
- You hold a licensed healthcare profession that the employer deems relevant to this position, with an interest in orthopaedics and emergency care.
- You have extensive experience in the care chain on a hospital ward.
- You can handle multiple tasks in parallel and see it as a challenge to work at a varied pace.
- You are attentive to patient needs and enjoy collaborating with people from different professional categories.
- You actively contribute to high patient safety and a good working environment through your knowledge and engagement.
