About the workplace

The Assistance Unit consists of 14 dedicated social secretaries, a social administrator, two senior social secretaries, and a unit manager.

The unit is responsible for authority exercises according to the Social Services Act (SoL) (elderly care) and the Support and Service Act for Persons with Certain Functional Impairments (LSS), as well as laws regarding transport assistance and national transport assistance. In addition to authority exercises, we actively work with collaboration and development in various areas, and your role as a social secretary is very important. You are offered competence development, internal method guidance, and external process guidance.

The Assistance Unit is located in central Nödinge, close to both the E45 highway and good public transport connections.

In this advertisement, we are looking for two employees for authority exercises in elderly care, primarily working with the target group aged 65+. One of the positions will mainly focus on the admission and discharge processes related to inpatient care. Job Responsibilities

As a social secretary, you process applications for assistance and requests for interventions according to the Social Services Act (SoL), primarily concerning interventions related to the target group aged 65+. You work based on current legislation to inform, investigate, assess, make decisions, and follow up on interventions. In your role as a social secretary, you will meet people with various disabilities, their families, networks, as well as guardians/administrators. We collaborate with service providers, other social services, authorities, and healthcare. There is a possibility that handling transport assistance cases according to the Transport Assistance Act is included as part of the assignment.

The work involves a high extent of authority exercises with requirements for legal certainty, a common approach, and equal assessments. With the help of your knowledge and good customer service, you must ensure that the individuals we meet receive the help and support they need to live as independent a life as possible based on their own conditions. As a social secretary in our SAMSA team, a large part of your assignment is planning and coordination aimed at working for a safe and smooth discharge from inpatient care. This takes place in close dialogue with the individual, their relatives, and relevant professions. Within the team, you follow the individual's path through the discharge process - from inpatient care to a safe return home, where the area case manager takes over.

The work places high demands on collaboration, structure, and holistic view. You collaborate closely with inpatient care, primary care, and municipal care and nursing to ensure that the individual's needs are met in the best possible way - in accordance with the Social Services Act and with the individual's best interests in focus.

The future demands new ways of working and shared responsibility in the planning chain. Here, you become an important component in this work and part of an engaged team.

Qualifications

We are looking for you who are educated as a social worker.

We would like you to:

- Have experience with authority exercises regarding the current target group and be well acquainted with current legislation.

- Have good communicative and analytical skills, both internally and externally.

- Be confident, structured, and able to maintain focus on the assignment despite the various demands of the surroundings.

- Enjoy collaborating and contributing to joint solutions.

- Like development and are driven by finding efficient and legally secure working methods.

It is important that you have good system proficiency and experience with the business system Treserva. Experience with SAMSA and IBIC is meritorious.

A driver's license (Category B) is a requirement.
