Rättvik Health Centre has approximately 10,300 registered patients and a staff of around 55 employees. Here, various professions work together in close collaboration. We have well-functioning specialist nurse clinics in the areas of diabetes, hypertension, addiction, dementia, as well as general nursing clinics, care coordination, and coordination with our partner healthcare providers. This autumn, we will begin developing a new way of working within primary care.

Both nationally and locally, there is a need to develop primary care practices to meet increasing healthcare needs, improve continuity, and create a sustainable work environment. This involves creating conditions for increased continuity by ensuring that each patient belongs to a cross-professional team with shared responsibility.

This approach strengthens the ability to offer a designated healthcare contact, increase patient safety, and create security for both patients and staff. Continuity is based not just on an individual employee, but on a team that collectively possesses knowledge of the patient's needs and can ensure cohesive care.

We have the opportunity to strengthen our clinic and are looking for a dedicated nurse.

You will work with competent and friendly colleagues where our patients are always the focus. As a nurse here, you will have a varied and meaningful workday involving both clinic duties and acute assessments. You will work independently but in close collaboration with colleagues from different professional backgrounds.

Main responsibilities:

• Clinic duties, telephone advice, and 1177 digital advice.
• Assessment and handling of acute patient cases.
• Follow-up of patients with chronic diseases.
• Teamwork and coordination regarding patient care.
• Participation in improvement and development work.
• Opportunity to work in specialist clinics based on operational needs and interest.

Employment will be arranged according to agreement. Selection is ongoing. A good introduction is provided.

Requirements:

• Registered Nurse (RN).

Merits:

• Specialized education as a District Nurse.
• Experience in the nursing profession.
• Experience from primary care, emergency care, or home healthcare.
• Interest and/or knowledge of coordination with municipal nurses, social secretaries, and other actors.

Personal qualities:

You thrive in a role with varied tasks and can handle both planned and acute situations. You have a genuine interest in coordination and close collaboration with municipal healthcare providers.
