In preparation for the transition to the new Social Services Act and "Nära vård" (Close Care), municipal primary care is a vital resource. New thinking and new ways of working need to be implemented – that is why we need you, who want to be part of developing future care and support! We have already begun implementing new working methods, and this autumn we are looking for a new nursing colleague who wants to help develop home healthcare.

About the role

We are looking for you who are a registered nurse and want to work in home healthcare in Borgholm. Within the Health and Medical Care Unit, you will be part of a positive and engaged group of nurses working to develop care in the patient's home environment. You will be responsible for patients in ordinary housing and/or in care and support residences.

About the Social Services Administration

The Social Services Administration is responsible for the municipality's social care, including:

• Elderly care
• Municipal health and medical care
• Individual and family care
• Care for people with disabilities
• Administrative unit

Job Responsibilities

As a nurse within the health and medical care operations in Borgholm Municipality, you will work together in multidisciplinary teams with occupational therapists, physiotherapists, unit managers, and care staff. The health and medical care operations are divided into three geographical team areas: north, middle, and center. The teams work with both ordinary housing and special housing.

In Borgholm Municipality, the health and medical care unit has a well-established collaboration with Borgholm Health Center/Home Hospital and the ambulance service.

The work consists of routine as well as advanced tasks for a nurse within home healthcare and elderly care.

Requirements and Qualifications

• Registered Nurse/Community Nurse, preferably with additional education or experience within home healthcare, elderly care, palliative care, psychiatry, or open health and medical care.
• Confident in the nursing role with professional experience within health and medical care.
• Ability to work both independently and in a team.
• Category B driving license is a requirement.
