Outpatient Care and Outreach

Description

We are looking for an engaged and professional Therapist for outpatient care who wants to contribute to improving the quality of life for the residents of Perstorp.

The new Social Services Act implies an increased focus on early, preventive, and accessible interventions for children, young people, and families. In our transition work, we will meet people where they are to a greater extent and offer support before problems grow large.

We aim to provide the right treatment and support at the right time for each individual. Within our unit, we work with both interventions without needs assessment and benefit-assessed interventions, with a strong focus on preventive and early interventions to reach residents in the municipality who need support at an early stage. Interventions are given both individually and in groups, and the unit also provides various types of meeting places.

We are now looking for engaged therapists who want to work with intensive treatment at home and contribute to strengthening families' own resources in their daily environment. The work is characterized by flexibility, accessibility, and high intensity based on the family's needs.

Job Responsibilities

In the role of outpatient therapist, you will play a central role in supporting and strengthening the residents of Perstorp. You will work with:

  • Conducting individual assessments, creating personal treatment plans, and qualified treatment interventions in families' home environments based on each individual's needs and conditions.
  • Working with change work together with children, young people, and guardians both individually and in groups, strengthening families' ability to handle daily challenges.
  • Leading and participating in conversation groups and activities aimed at promoting mental health and quality of life.
  • Collaborating with schools, healthcare, police, and other important actors to ensure cohesive care for individuals.
  • Working preventively to reduce the need for more intrusive interventions.
  • Documenting and following up on interventions according to current legislation and the organization's guidelines.

Qualifications

We are looking for you who are a social worker, social pedagogue, or equivalent higher education that the employer deems suitable.

You have experience in treatment work with children, young people, and families.

You are confident in working independently while also valuing collaboration.

You have the ability to create trustful relationships even in complex situations.

You have a good ability to document interventions in accordance with current legislation.

It is meritorious if you have education in, for example, DBT, CBT, MI, or other evidence-based treatment methods.
