Bjärke Home Care covers a large catchment area and consists of two groups that collaborate. We operate from newly built premises in central Sollebrunn. Our catchment area includes central Sollebrunn, Gräfsnäs/Långared, and Magra/Mellby. In central Sollebrunn, we primarily use bicycles as transport, while in Gräfsnäs/Långared and Magra/Mellby, we drive cars.

Job Description

In your role as an assistant nurse, you will work independently but also collaborate with your colleagues in your work team and, if necessary, with nearby work teams to achieve the organization's goals. You will work based on the Social Services Act (SoL) and perform delegated tasks according to the Health and Medical Services Act (HSL).

The tasks vary depending on the care recipient's needs. You will primarily work with good care, service, and social interaction, as well as documentation, planning, health and medical care tasks, and development work. The work is performed in the care recipients' homes, and we cycle or drive between visits.

As an assistant nurse, you need to be flexible and responsible, as workdays can range from handling alarms, medication management according to prescription, and daily care to assignments as a permanent contact person. Together with your colleagues, you play an important role in creating security, participation, and good quality of life for the care recipients.

You also have an extended responsibility as a permanent contact person for a limited number of care recipients. In this assignment, you have a clear responsibility for coordination and continuity in the contact with the care recipient. You are often the first person to meet the care recipient after a granted decision and are responsible for welcome conversations, establishing, and following up on implementation plans. You collaborate with relatives and other professionals and document according to current legislation in the operational system.

There has also been significant development in digitalization for home care in recent years, and we work predominantly through our work mobiles. In the mobile, staff see their daily schedule, can sign for medications, and document important events in our journal system.

Qualifications

We are looking for you who:

  • Are trained as an assistant nurse with a protected professional title
  • Are well-versed in digital platforms, following the workplace's action plans, policies, and guidelines, as well as paragraph SoL §1:1 of the Social Services Act
  • Hold a B driver's license

Merits:

  • Supplementary education that the employer deems relevant
  • Experience working in elderly care with a dementia or psychogeriatric focus
  • Experience in other relevant assignments or representative roles as an assistant nurse

Employment type: Permanent position.
