Forsåker Health Centre and Child Health Centre (BVC) is a growing health centre in Mölndal, focusing on teamwork, quality, accessibility, and patient interaction. Here, the team succeeds, not the individual. We aim to be the local health centre with a big heart. We are now looking for the right person to join us in an exciting and fun phase. The health centre is run by three general practice specialists. Here, doctors, psychologists, specialist nurses, assistant nurses, medical secretaries, rehab coordinators, and administrators work together. We have clinics for patients with obesity, asthma/COPD, diabetes, hypertension, as well as a senior clinic. At the BVC, families meet both child health nurses and specialist doctors. We also work closely with our rehab clinic.
We believe in working thoughtfully, using smart IT systems and digital care when needed. We are HBTQ-certified. We strongly believe in continuity and ensure that our patients see the same healthcare staff. We work closely together as a team and share responsibility for the care of our approximately 5,800 patients. All to ensure that those who need care the most receive it.
We now need to strengthen our team with a Rehab Coordinator. Since we are a growing operation, it is important to us that you are flexible and willing to help us develop the clinic's work forward. About the position
As a rehab coordinator, you focus on facilitating return to work, preventing, and optimizing sick leave from a patient perspective. The task is primarily to coordinate the measures deemed valuable for the development of a quality-assured, uniform, legally secure, and gender-equal sick leave and rehabilitation process within your own health unit and for patients with reduced work capacity. We are working on a sick leave project where we plan to work sustainably and cross-professionally. The rehab coordinator will be an important team player. We are looking for someone part-time; the percentage of employment can be discussed depending on the primary profession.
The assignment includes three main areas:
- Individually adapted support to patients by, among other things, identifying and mapping the patient's rehabilitation needs and, at an early stage, identifying patients who need coordinating efforts.
- Collaborate and coordinate internally by coordinating and developing internal work within the sick leave and rehabilitation process.
- Collaborate externally by being the contact person for external actors such as employers, occupational health services, etc.
To fit the role of rehab coordinator, you must:
- Have good cooperation skills and experience in teamwork
- Be driven and proactive
- Be administratively skilled
The work is done in close collaboration with doctors and other colleagues. Experience with Webdoc and Visiba is not a requirement.
About you
The primary profession is not the most important factor. You are a licensed healthcare professional and trained as a rehab coordinator. Great emphasis will be placed on personal suitability. We welcome your application; selection and interviews are conducted on an ongoing basis. Start date by agreement, with a target for autumn.
