The Mobile Healthcare Team provides home care for patients who have difficulty traveling to their regular healthcare provider. The team is organizationally part of Primary Care (VO Primärvård) in Region Gävleborg. The operations primarily rest on two pillars: we conduct more or less acute home visits as a mobile team consisting of a doctor and a nurse, and we are responsible for home healthcare in Hudiksvall Municipality. We work broadly and collaborate with all health centers in the area, both corporate-owned and privately run, as well as with municipal healthcare and the operations at Hudiksvall Hospital. Our three cars are on the road Monday–Friday with doctors and nurses in Hudiksvall and Nordanstig to create professional and secure home care.

We are now looking for a General Nurse for the Mobile Healthcare Team in Hudiksvall/Nordanstig. The position involves 20% rotation to inpatient care based on previous experience and competence.

Job Description

The work of the Mobile Healthcare Team is based on a holistic assessment of the patient's situation in their home environment. Through team assessment and coordination of resources, it is ensured that the patient receives the right interventions at the right time. The work involves, among other things, contact with patients, relatives, and other healthcare providers to plan home visits and handle incoming patient cases.

As a nurse with us, you will:

• Receive and distribute incoming patient cases.
• Plan and book home visits.
• Collaborate with various healthcare actors.
• Make own assessments and treatments of patients with support from a doctor.

Qualifications

We are looking for you who are proactive and confident in your professional role, with the ability to independently plan and prioritize work based on patient needs. You are flexible and adapt easily when conditions change in daily work. Since the work is based on close collaboration with both colleagues and other actors within healthcare, we see that you have good cooperation skills and contribute to a positive and professional work climate. You see opportunities in changes and are eager to contribute to developing methods that strengthen care for our patients.

In the role of nurse, you must:

• Be a licensed nurse.
• Have good knowledge of Swedish, both spoken and written.
• Hold a valid Class B driver's license.
