Occupational Therapist for Care Planning Team in Borlänge
Borlänge Municipality's rehabilitation department is a cheerful group that supports each other, and we are now looking for you who want to become part of our good work environment. We are currently seeking an occupational therapist to join our care planning team and contribute to safe care transitions and the development of close-to-home care.
Here, you will work in a cross-professional team together with nurses and assistance officers, where you are part of both the care planning team and the entire rehabilitation group. In your work, you contribute your competence to care planning while collaborating closely with other professions within rehabilitation, such as MAR (Rehabilitation Assessment and Rehabilitation), rehabilitation assistants, and administrators, to create holistic and high-quality care.
The municipality's rehabilitation activities are broad and cover ordinary housing, special housing, short-term housing, respite care, as well as interventions within disability services and social psychiatry. You will have a varied, developing, and enjoyable work environment where you take significant responsibility, while always having colleagues there to support you.
As a new employee, you will participate in our introduction program with supervision. The standard introduction period is four weeks but is adapted individually based on your needs. Everything is designed to ensure you have a great start with us, feel comfortable, and secure.

Job Responsibilities
As an occupational therapist in the care planning team, you work with discharge planning for patients who are ready for discharge from the hospital. You gather and compile information about the patient's needs and resources to plan the continued care within the municipality in collaboration with the team's nurses and assistance officers. Together, you contribute to creating safe and secure care transitions between inpatient care, municipal care, and outpatient care.
The work involves close collaboration with the region's hospitals, primary care, and rehabilitation clinics. All planning and coordination take place digitally in Cosmic Link, where dialogue and information exchange are a natural part of daily work.
The position includes both indirect and patient-facing tasks with a focus on planning, coordination, and professional dialogue. As part of the transition to close-to-home care, you will work together with your colleagues to develop and follow up on working methods within the care planning team.
Your primary employment is as an occupational therapist within the municipality, which means patient-facing work may also occur outside the specific assignment in the care planning team. The role thus gives you a variation between planning work and clinical work close to the patient.
You will become part of a developing operation where collaboration, structure, and engagement are crucial, and where you have the opportunity to influence future working methods.
Qualifications
We are looking for you who are a licensed occupational therapist. You have experience from patient-facing work as an occupational therapist within municipal primary care, inpatient care, and/or outpatient care.
You have good ability to express yourself in speech and writing. Your ability to be communicative, receptive, and solution-focused makes it easy for you to create and maintain good relationships. Having a good attitude towards people is a given for you, whether you meet patients, relatives, colleagues, or partners. You have a very good ability to collaborate and cooperate with many different actors in a trustworthy and professional manner.
You enjoy working in a team and have a consultative working style. You need to be able to independently plan, structure, and prioritize your work, being careful yet efficient. We also expect you to be engaged, show initiative, take own initiatives, and contribute to job satisfaction.
Good familiarity with working in various healthcare-related IT systems is a prerequisite for fulfilling the assignment. A Category B driver's license is required.
A certificate of good conduct (extract from the criminal record) will be requested before employment. Interviews will be conducted on an ongoing basis.
Salary: Monthly salary.
Employment type: Permanent position.
Duration: Permanent.
Working hours: Daytime.
