Welcome to us
The Health and Medical Care function (HS) in Jönköping Municipality consists of 13 cross-professional HS Teams, as well as a Home Team. HS has specializations within psychiatry, palliative care, home rehabilitation, dementia, and functional support. Together, we are approximately 460 dedicated employees.
HS-Team 10 is part of the Health and Medical Care function, covering the areas of Grästorp, Hovslätt, Norrahammar, Taberg, and Månsarp, including the surrounding countryside. Within the area, there are three nursing homes: Smedjegården, Gjutaren, and Fridhälls. The work group consists of 20 employees in the professions of nurses/specialist nurses and assistant nurses. We operate from our team premises in Norrahammar. We work cross-professionally together with occupational therapists and physiotherapists.

Your new job
As a nurse, you are responsible for both planned and acute health and medical care interventions in the individual's home, regardless of housing type. You make independent medical assessments and perform treatments and procedures. Here, you meet patients of various ages and collaborate closely with both primary and inpatient care to tailor the care intervention to the individual. The work is based on person-centered care, which means giving the patient influence and the opportunity to take personal responsibility and experience security in their home.
With us, you will follow the patient over a longer period and contribute to continuity in care. You also have the opportunity to participate in development and quality work. The role involves mentoring and teaching other care personnel, as well as patients and relatives, to ensure high quality in care.
Your competence
- You are a registered nurse. Specialist education such as elderly care, palliative care, psychiatry, or district nurse education is considered meritorious.
- We would appreciate if you also have experience working in care and social services, for example as a nurse or assistant nurse.
- You have good knowledge of Swedish, both spoken and written, as well as good computer skills.
- You have good physical mobility as the work takes place in a home environment, and you need to adapt your working method to the existing conditions.
- A B-class driver's license and driving experience are required as a car is used for work.
As a person, you are empathetic and receptive, with clear, understanding, and respectful communication that creates security for patients and relatives. You are flexible but still work structured even under changed conditions. You value good collaboration and act with both the individual's and the organization's best interests in focus. To ensure patient safety and well-being, you have good judgment, are meticulous, and can translate your knowledge into practice.

Good to know
The recruitment is for a permanent full-time position with start date as agreed. Working hours are primarily during the day, with evening and weekend work included to some extent. We work according to a flexible schedule with the possibility to influence your scheduling, and currently, we work two weekends in a ten-week period.
