Job Responsibilities

Do you want to work closely with patients, make your own decisions, and make a real difference every day? We are looking for a committed and confident nurse for home healthcare in Bomhus and Brynäs.

Here, you will meet patients in their home environments and become an important part of their sense of security and daily life. The work is varied, developmental, and independent, while you always have the support of colleagues in the team.

As a nurse in home healthcare, you are responsible for the planning, implementation, and follow-up of health and medical care interventions in the patient's home. You lead the nursing work and are a central part of the coordination around the patient.

Your duties include, but are not limited to:

• Medical assessments and nursing responsibility.
• Medication management and delegation.
• Documentation and follow-up.
• Collaboration with healthcare providers within the region and municipality.
• Mentoring and support for care staff.

You plan your workday independently and prioritize based on patient needs. The role offers both responsibility and freedom.

The position is primarily day-time work, but the schedule includes evening and weekend shifts. During these times, you will work in our consultancy service, which includes the municipality's care and nursing homes, home healthcare, and service and group housing within LSS (Act for Support and Service for Persons with Certain Functional Impairments) and social psychiatry.

Interviews may be conducted continuously during the application period.

Qualifications

You are a confident and clear nurse who builds trust in interactions with patients, relatives, and colleagues. Through your calmness and professionalism, you lead nursing work with structure, handle changes with stability, and contribute to an open and developmental collaboration within the team.

To be considered for this role, you must have:

• Nursing license (Legitimation).
• At least 2 years of experience as a nurse, preferably within municipal health and medical care.
• Valid driver's license and driving experience.
• Experience with documentation systems.

Experience with the following systems is considered a merit: Treserva, TES, Lifecare, NPÖ, and Pascal.

Employment Type: Permanent position.
