The Social Services Administration, with approximately 1300 employees, encompasses five operational areas.

The Social Services Administration offers meaningful and significant jobs. Our aim is that care and support should be based on collaboration between the individual, relatives, and employees.

The ordinary housing operational area is now seeking a planning coordinator for Österfärnebo.

JOB RESPONSIBILITIES

Job responsibilities included in the role;

  • Plan and optimize work schedules for home care staff based on the needs of care recipients and staff availability.
  • Manage urgent changes in planning and staffing in case of absence.
  • Communicate and collaborate with colleagues, care recipients, relatives, and unit managers.
  • Follow up and document work to ensure quality and efficiency.
  • Manage interventions without individual needs assessment.

As a planning coordinator, you are responsible for planning and coordinating the daily work within home care. You maintain contacts with care recipients, relatives, case managers, and licensed staff. The planning coordinator has a supportive role in the organization to enhance the quality of care work with the aim that guidelines and routines are followed in the organization. New is that you will handle certain interventions that do not require individual needs assessment (interventions that were previously decided by case managers).

The planning coordinator is a culture bearer where good quality, good reception, and efficiency are central parts of the work. The planning coordinator should, in close collaboration with the unit manager, analyze and possibly redistribute resources and have an economic mindset for the most optimal planning.

The planning coordinator assignment is a part-time position to varying degrees, the right to full-time means a combined position with care work in home care.

In the role of care work within home care, you provide care and social support based on the individual needs of care recipients, in their own homes. You assist the care recipient in daily life as desired. You contribute to the experience of security through a responsive, smooth, and professional reception. Your work includes establishing individual implementation plans, social documentation, and assisting with daily rehabilitation.

The position is daytime Monday to Friday.

QUALIFICATIONS

Your qualifications are;

  • Experience in planning work in home care.
  • Good IT skills.
  • Strong communication skills and the ability to work in a structured and solution-oriented manner.
  • Good knowledge of Swedish, both spoken and written.

Personal suitability;

For the role of planner within home care, personal suitability is a decisive factor. We are looking for someone who has a natural aptitude for structure, accuracy, and problem-solving. You have the ability to keep a cool head and act flexibly even under time pressure or when conditions change rapidly.

Furthermore, you are attentive and have good communication skills, which allows you to build good relationships with colleagues, care recipients, and relatives. As a person, you are responsible and committed to contributing to an organization that promotes both quality and care.

Finally, you appreciate collaboration but also have the ability to work independently and take your own initiatives to improve planning and effectively solve emerging challenges.

If a combined position is relevant, it is important that you are a nursing assistant and want to make a difference. You are attentive, empathetic, and professional. You maintain focus and are calm, accurate, and methodical in your working approach when you are out with care recipients. The knowledge and competence required for a nursing assistant in elderly care are described in the functional description for care staff.
