Specialist nurse with a focus on district nursing for our growing home care assignment in Bromma

We are looking for someone with good structure and leadership skills, who feels secure in the role of a nurse – a person who wants to continue developing and contribute to the future primary care of Region Stockholm. At Bromma Medical Center's home care assignment, we are a team of 3 nursing assistants, a full-time district nurse, and a home care doctor at 20%. We are a dedicated and highly competent team of doctors and nurses who together run a smaller, personal health center in the middle of Brommaplan.

Our goal is to provide care of the highest quality supported by the latest tools and methods in modern healthcare.

About us

At Bromma Medical Center, we conduct a general practitioner assignment on behalf of Region Stockholm.

We have a growing home care assignment, active chronic disease clinics, and an attractive first-line assignment for adults.

As a smaller health center with close collaboration between professions, we create good continuity and high care quality.

Job responsibilities / Areas of responsibility

What does the role of a district nurse in home care entail? You will:

• be responsible for quality control visits to our home care staff.
• act as a coordinator for the assignment's three nursing assistants and be the first contact for clinical questions about the patients.
• perform daily checks of LifeCare and our direct phone number for patients aged 75 and older.
• act as an intermediary between the home care assignment and other stakeholders such as patients' close relatives, guardians, external case managers, etc.
• work closely with the home care doctor and management for clinical assessments and planning of the home care assignment.

Daily tasks

• Daily rounds with home care nursing assistants.
• Visits and blood tests for the health center's home care patients, possibly SIP visits.
• Control of LifeCare.
• Staffing our direct phone number for patients aged 75 and older, 1 hour per day.

Main goals and challenges of the role

• Provide safe, continuous, and evidence-based care.
• Gather and structure sometimes complex care needs for patient-safe care.
• Create continuity in home care quality.
• Coordinate care for patients with complex needs.
• Contribute to the development and good quality of the operations.

Qualifications / Requirements

• Completed district nurse education with previous experience in basic home care.
• We are looking for someone with good structure in their work, who gathers colleagues around them and drives the group forward with goal-oriented teamwork and good collaboration and communication skills. You are empathetic and responsive in patient interactions, structured and responsible with good judgment, proactive and development-oriented, and a team player who works with a positive attitude without prestige.
