The organization is part of Close Care and Health, Mobile Teams, and Specialized Palliative Care in southwestern Skåne. ASIH offers specialized palliative care and other advanced medical care at home to patients of all ages residing in Malmö's catchment area. Care is provided in patients' own homes or in special accommodations.
The patient's needs guide the visits; we perform medical and nursing assessments and actions. We work independently but are part of interdisciplinary teams consisting of doctors, nurses, social workers, physiotherapists, occupational therapists, nursing assistants, dietitians, and medical secretaries. We have our own on-call line, and the team's doctors are available around the clock.
ASIH has full medical responsibility for enrolled patients and works with holistic, person-centered care, which is a prerequisite for providing dignified care at the end of life. ASIH is responsible for specialized care but collaborates with home care when needed and home service for basic nursing. ASIH will expand to three teams and have a total of 60 care places starting from 2026-03-02. Additionally, we have an outpatient team consisting of a palliative clinic and a point intervention team that provides interventions for patients not enrolled in ASIH.
JOB RESPONSIBILITIES
The patients enrolled in the organization require specialized palliative care, which means they have a complex symptom profile or that their life situation entails special needs. This can involve everything from injections, infusions, medication pumps, catheter placements, ascitic tapping, oxygen, and dressings to supportive conversations, family support, or assistance in contact with authorities, among other things.
After planned patient visits, afternoons are used for documentation, reflection rounds, team supervision, individual reflection, and development work. Additionally, there is a handover to those working the evening shift and planning for the next day. Those working the evening shift then have several planned patient visits and are responsible for any emergency visits. ASIH's phone is open around the clock, and we constantly assess and make any visits to patients who need it. Besides the daily work, as a nurse, you will have admission conversations with doctors, participate in assessments for potential admissions, and continuously work with family and bereavement support. We work both day, evening, and night shifts.
As a new nurse with us, you will receive at least four weeks of shadowing; we follow an introduction program developed at the unit, and after completing the shadowing, you will have mentorship where you receive support from a specialist nurse in palliative care, according to your needs. We value development work and continuing education for both new hires and experienced staff. ASIH has a close collaboration with the palliative care department, which has 12 inpatient care places. We cooperate with the hospital and various clinics.
At night, there are two nurses on duty. The shift begins with a report from the evening staff. Then any planned patient visits and emergency visits are performed. The nurses at night are also responsible for preparing materials and medications for the next day's home visits. The shift ends with a report to the ongoing staff. Night shifts are from 10:00 PM to 8:00 AM.

QUALIFICATIONS
We are looking for a licensed nurse with the authority to practice within Swedish healthcare, preferably with a few years of professional experience. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's level scale are a requirement. Since the organization consists of care places in the patient's own home or in special accommodations, a valid B driver's license for a manual transmission vehicle is a requirement. We would also like you to have specialist training in oncology and/or palliative care.

OTHER
The Close Care and Health administration has existed since January 1, 2025. The administration is responsible for grant-funded care activities within close care. This includes, among other things, mobile teams and specialized palliative care, outpatient care within close care, emergency care within close care, 1177 by phone, and some region-wide functions. Additionally, the administration is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
