About the role

As a case officer for the elderly according to the Social Services Act (SoL) at Ideal Bemanning, you will:

• Be responsible for investigating and assessing the needs of the elderly for support according to the Social Services Act (SoL).
• 
  
• Conduct investigations and follow-ups in close dialogue with elderly individuals and their relatives.
• 
  
• Participate in the coordination and planning of services in collaboration with other professionals, such as nurses, occupational therapists, and physiotherapists.
• 
  
• Work preventively and supportively with a focus on creating a safe and functional everyday life for elderly individuals.
• 
  
• Document and follow up on services according to applicable guidelines and legal requirements.
• 
  
• You will have the opportunity to work in a varied and developing environment, where each day offers new challenges and meetings with elderly individuals and their families.

We are looking for someone who

• Has a degree in social work or another relevant education.
• 
  
• Has at least 3 years of experience in elderly care and case management according to SoL in the last 7 years.
• 
  
• Has a genuine commitment to supporting elderly individuals in maintaining a good quality of life and independence.
• 
  
• Is confident in your role as a case officer and has the ability to work independently as well as in a team.
• 
  
• Has good communication skills and an empathetic approach towards elderly individuals and their relatives.
