The authority function consists of about sixty employees who work in pleasant premises at Lund Central Station.

The unit for the elderly in ordinary housing includes three workgroups and a method and development leader. The workgroups consist of case officers for ordinary housing, case officers directed towards special housing, and the SVU team (collaboration during discharge from inpatient care). The SVU team is a cross-professional team consisting of nurses, case officers, and occupational therapists. The team is responsible for planning support for the individual before discharge from inpatient care. The professionals in the team decide based on their assignments on the need for assistance according to the Social Services Act (SoL) and interventions according to the Health and Medical Services Act (HSL). The board aims to work on preventive and rehabilitative/habilitative efforts by 2025. This is to increase independence and delay the need for care and support services.

Job Responsibilities

As an occupational therapist, you work to create conditions for the individual to live as independently as possible after hospitalization. Together with your colleagues, you plan for discharge from the hospital, which may involve personal meetings at the hospital, digital planning, or phone planning. Collaboration with internal and external operations is a daily occurrence.

Collaboration and coordination are a significant part of the work, and being able to cooperate with other professional categories is essential. You will participate in the development of routines and collaboration both within internal and external operations. Working for high quality with satisfied users and a good work climate is an important part of the assignment and something we believe motivates you.

Qualifications

We are looking for a licensed occupational therapist, preferably with a few years of experience in the profession. As an employee, you have good collaboration skills and are flexible, easily adapting to changing circumstances in your work. You should be independent, solution-focused, and actively participate in our operations.

As a person, you find it easy to collaborate with others and create good working relationships. You can take on a task in an initiative-driven and self-sufficient manner. You have a good ability to express yourself verbally and in writing. We expect you to have good organizational skills, knowledge in administration and documentation. You possess social competence, empathy, and responsiveness in contact with applicants.

Experience in planning for collaboration during discharge is an advantage.
