Coordinator, pain rehabilitation, Vårdcentral Mora Job Responsibilities

Vårdcentral Mora is now seeking a care coordinator/rehab coordinator who wants to help develop and work in our multimodal team for patients with chronic pain. Region Dalarna is undergoing a reorganization to improve care in this area, where Mora is one of three health centers in the region receiving an expanded mandate. The team will offer multimodal rehabilitation for chronic pain to patients registered at health centers in Northern and Western Dalarna. This team includes care coordinators/rehab coordinators, physiotherapists, occupational therapists, doctors, and psychologists. This position primarily involves coordinating and organizing care for patients enrolled in the team. As a care coordinator/rehab coordinator at Vårdcentral Mora, you will work both independently and in a team with other professionals both internally and externally. You will lead rehab meetings, document care plans, and follow them up together with the team. You are simply the spider in the web! In addition to working in the chronic pain team, you will also work with your profession's usual tasks within primary care. Depending on your competence and interests, there are opportunities to work with or develop within various areas/assignments of primary care. With us, there are colleagues with extensive experience and knowledge in many different areas, providing you with security in the primary care assignment, and of course, an introduction will be offered.

Requirements:

• Licensed nurse, occupational therapist, physiotherapist, or equivalent basic profession for the assignment with a genuine interest in primary care assignments and chronic pain.

Meritorious:

• Previous work in primary care, psychiatry, or pain rehabilitation.
• Experience in coordination or rehab coordination.
