Licensed Physiotherapist for the Home Team in Jönköping Municipality

We are now looking for a licensed physiotherapist who wants to become part of our dedicated home team! With us, you will meet people in various life situations and contribute to safe care in a home environment.

The home team investigates, assesses, and plans interventions for new or changed care needs after hospital stays. We work in interdisciplinary teams with the goal that citizens can manage on their own as much as possible and feel safe and involved in their daily lives. The interventions usually last for two weeks, after which we report to home care, health centers, or our regular health and medical care teams for continued rehabilitation and care.

Your New Job

As a physiotherapist in the home team, you will meet citizens in their homes after hospital stays. You will independently conduct investigations and assessments and plan interventions based on the citizen's own abilities, resources, and needs. You will collaborate with inpatient care to create a safe transition from hospital to home.

Your role also includes providing advice and support, guiding and educating citizens, relatives, and other professionals. Documentation is an important part of ensuring good and safe care.

The work is varied as you meet citizens of different ages with diverse needs and diagnoses.

Your Competence

Requirements

• Licensed physiotherapist
• Driver's license and experience driving a car
• Good knowledge of Swedish, both spoken and written
• Good computer skills for documentation

Meritorious

• Experience working in municipal health and medical care
• Experience with the Combine operational system

Welcome to Us

The health and medical care function (HS) in Jönköping Municipality consists of 13 HS teams and one home team. HS has specialist areas in psychiatry, palliative care, home rehabilitation, dementia, and functional support. Together, we are about 460 dedicated employees.

The home team is organized within elderly care, and the licensed staff are employed in the health and medical care function. The workgroup is based at the team location on Klubbhusgatan in central Jönköping and consists of nursing assistants, physiotherapists, occupational therapists, nurses/specialist nurses, and assistance assessors.

Good to Know

The recruitment concerns a permanent full-time position with a start date according to agreement. Working hours are scheduled during the day, but some weekend work during holidays may occur. Jönköping Municipality works with full-time as the norm and always offers 100% employment.
