We are looking for a nurse/community nurse for our clinic

Kista Health Center has over 11,300 registered patients and is located in the middle of Kista Galleria. It takes about 17 minutes from the city center by subway, and there are good bus connections.

At the health center, we have doctors, community nurses, nurses, nursing assistants, medical secretaries, receptionists, psychologists, and a rehab coordinator. We are about 37 people working together to take care of our patients in teams. You can read more about us at Kista Health Center.

We are one of four health centers that also have an SMO assignment, Coordinated Medical Care for patients with permanent disabilities.

Job responsibilities:

With further education and in-depth competence in diabetes, you will be responsible for receiving and following up patients with diabetes at the clinic. You will work with counseling, treatment, and continuous follow-up to support patients in their self-care and contribute to safe and secure diabetes care. In primary care, you will meet patients of various ages and with varying health conditions at the clinic, both scheduled and urgent visits. The assignment also includes regular tasks such as clinic work, triage, telephone counseling, and if necessary, also within home healthcare. The work requires broad competence and independence.

Documentation is done in Take Care, and we use the telephone system TeleQ.

Employment form:

Permanent employment. Full-time. If you have requests for a lower working percentage, this can be discussed. A probationary period may be applied. Working hours Monday-Friday. Start date according to agreement.

Qualifications:

Community nurse with prescribing rights and education in diabetes care or a licensed nurse with specialist training in diabetes care. You have the authority to work within Swedish healthcare.

Experience working with diabetes and in primary care is highly meritorious. We would also like you to have experience with the TakeCare journal system.
