Our mobile team in Torsby is called specialized home healthcare hospital Torsby and is organized under the medical clinic.

A merger has taken place this spring between the Mobile Primary Care Team and the Palliative Team. The catchment areas are the municipalities in northern Värmland, namely Torsby, Sunne, Hagfors, and Munkfors. The unit is well-functioning and consists of doctors, nurses, physiotherapists, and social workers, and they have close collaboration with municipalities, hospitals, and primary care. The team works daytime on weekdays.

The development work towards Good and Close Care lies ahead of us in the team, where the core is a person-centered approach. We know that increased collaboration between caregivers contributes to increased safety and accessibility for patients and relatives. The sense of community is constantly present at Torsby Hospital, and all units play a central role in the work. We are a small hospital in northern Värmland with a big heart and commitment to our patients and employees, where all units and partners are highlighted as an important part of the hub for the work to flow smoothly and safely for those who visit us. Good collaboration between units, short decision-making paths, and close contact with hospital management are characteristics of us.

At the hospital, we care about the health and well-being of our staff, and we have access to our own staff gym as well as a personal trainer. We are looking for someone who wants to be part of our workgroup. Here, there is high competence and experienced colleagues.

We are now looking for a specialist physician with an interest in working in the team. The team has different focuses and patient groups, and a large part of the work consists of palliative care. The scope of the position can be discussed, and there is a great opportunity to influence and be involved in developing the work. Future training can be offered for the assignment. Your Responsibilities

The team offers specialized care to patients with complex care needs in a home environment. The care is individualized and needs-driven. As a physician in the team, you are responsible for creating, updating, and communicating care plans to patients and care neighbors. Current care levels, patient and relative conversations are important cornerstones.

Team rounds with medication reviews and status reviews occur regularly, as well as updates of medication lists. Home visits occur regularly, usually together with the team’s nurses. Home visits, examinations, assessments, prescriptions, conversations, and care plans are conducted together with the team’s nurses and other involved parties.

Your Qualifications/Competencies

We are looking for a specialist physician with a suitable specialty and experience. We see that you have an interest in working with and developing care for severely ill patients at home.

A mobile working method requires flexibility and good communication skills, as well as experience working with other professions. We require language skills equivalent to Swedish C1 or higher. A valid driver's license (B) is a requirement.

You should have good initiative, be able to easily start activities and achieve results. You should see opportunities for change and development. Furthermore, we see that you structure and plan your work effectively. You have the ability to coordinate and lead to achieve common goals.
