The mobile team for specialized home healthcare (SSIH) in Arvika is in an exciting phase of development.

The goal is to meet the future needs for accessible specialized home healthcare regardless of diagnosis, age, and residence - and to lead the transition towards Close Care.

We work with patients aged 18 and over who have extensive needs for care and treatment due to illness, injuries, or aging, as well as with patients with complex palliative care needs in the early or late stages of their illness. The overarching goal is equitable, needs-driven, and person-centered care.

We primarily work in the patient's home but also digitally and in close collaboration with the nurses in the municipality. We also work to develop our operations by going out on more subacute missions and providing shorter temporary interventions for individuals who are not enrolled in our services. This way, we facilitate for patients who can remain at home, creating a more seamless care experience in western Värmland. The work is done in close cooperation with home healthcare in the western municipalities of the county; Arvika, Eda, and Årjäng, health centers, as well as with outpatient and inpatient care. We offer you a meaningful job in a safe, developing, and learning work environment. Our team consists of doctors, nurses, and nursing assistants in collaboration with dietitians, social workers, and physiotherapists. Together, we create conditions for safe home care with optimized symptom relief and treatment during a time in life, or until the end of life, regardless of underlying illness. Everything is tailored to the individual patient.

Your Responsibilities

As a nurse in the team, you are the contact person for enrolled patients and their relatives. You conduct home visits and carry out temporary and permanent medical interventions, as well as serve as assessment support for the municipality's nurses.

Your work also includes, at the request of another healthcare provider, going out on temporary interventions and subacute missions for patients who are not enrolled with us. This is a development effort to provide good and close care for our most vulnerable residents. The position involves full-time day shifts.

Your Skills/Competencies

We are looking for a licensed nurse with an interest and curiosity for the transition towards Close Care. If you are a district nurse, have specialist training in palliative care or advanced home healthcare, it is considered an advantage. A driver's license is required as travel is part of the job.

You should have a few years of experience in the profession, be comfortable and confident in your role, and able to make assessments independently. You are good at problem-solving and can handle complex issues. It is desirable that you find it easy to establish contact, collaborate, and maintain good dialogue with other healthcare providers. Personal suitability is highly valued.
