The Hospitals in the West is an administration within the Västra Götaland Region and consists of four hospitals – Alingsås Hospital, Angered Local Hospital, Högsbo Local Hospital, and Kungälv Hospital. We welcome a nurse to our Stroke/Home Rehabilitation Team! The team was established in 2014 and is a well-functioning small team with close and good collaboration.

The team consists of an occupational therapist, physiotherapist, speech therapist, social worker, nurse, and doctor. The team works during the day on weekdays. The team has a close collaboration with the stroke unit. Our goal is to offer follow-up and rehabilitation in a safe home environment.

Job Responsibilities

The nurse has an important coordinating role in the team and communicates with the department, patient, and relatives before discharge from the stroke unit, for a well-prepared and safe return home.

In the home rehabilitation team, the nurse conducts one to two home visits per patient. These visits include follow-up of e.g. blood pressure and blood tests based on medical orders. The work is person-centered with a focus on information and discussions about the hospital stay, diagnosis, medication, and risk factors. An important part of the nurse's work is to have supportive and advisory conversations with the patient and sometimes relatives about lifestyle factors such as diet, smoking, alcohol, and sleep, to reduce the risk of relapse.

The role also includes follow-up visits at the Stroke and TIA clinic with physical or digital healthcare meetings and telephone advice.

The team continuously works to develop and improve the operations, and in this work, you will be involved and the team has great opportunities to influence how the operations develop.

Qualifications

We are looking for you who are a licensed nurse, have experience in stroke care, and are interested in discussing lifestyle habits with patients. You enjoy working in a team and being a team player while also working independently in your daily tasks.

You like to work person-centered and have the ability to instill confidence in patients and relatives at home. You are proactive and attentive. Great emphasis is placed on personal suitability.

A B driver's license is required.
