JOB DESCRIPTION

Nurse with Coordination Duties

As one of our coordination nurses is moving on to new challenges, we are looking for you who want to combine clinical work with a coordination role.

We are seeking a nurse for the Health and Medical Services unit, with 50% of your time dedicated to the nursing pool and 50% as a coordination nurse.

As a pool nurse, you will be a key resource in both supported living facilities and home healthcare. In your role as coordination nurse, you will serve as the link between the municipality's health and medical services and the region's inpatient and primary care. You will act as the first point of contact for medical and nursing-related inquiries from staff, patients, and relatives.

You will be responsible for monitoring and handling cases in LifeCare SP, as well as coordinating care interventions for residents in need of coordinated care. The role also involves assessing the need for home healthcare or self-care based on the current agreement between the municipality and the region. Additionally, you will support colleagues regarding ongoing care interventions and care levels.

The municipality's health and medical services are continuously developing, with increasingly advanced care being provided in patients' homes. This makes the work stimulating, developmental, and varied—no two days are alike.

Working hours are daytime, Monday to Friday, 08:00–17:00 or 09:00–18:00.

QUALIFICATIONS

We are looking for you who are a licensed nurse. Specialist education or work experience in district nursing, palliative care, elderly care, geriatrics, or dementia care is advantageous, as is experience with care planning, coordination roles, and working in LifeCare SP.

Your communication skills are important, so you must have good proficiency in Swedish, both written and spoken. You should also be comfortable with computers.

A valid Category B driver's license is required.

Employment type: Permanent contract.
