Job Description

Your role is to work coordinatively with patients on sick leave, ensuring care is aligned with other stakeholders such as employers, the Swedish Social Insurance Agency (Försäkringskassan), the Public Employment Service (Arbetsförmedlingen), and municipal social services. As a Rehabilitation Coordinator, you will provide support and advice to individual patients, conduct assessments of their rehabilitation needs, and manage SIP/care plans and meetings with employers. You will also support physicians in developing clear rehabilitation plans. A key part of the role is coordinating and collaborating both internally and externally to support patients and prevent or reduce sick leave.

Work Environment

We are a team of approximately 35 employees who thrive in an open and collaborative work environment. As a Rehabilitation Coordinator, you will work closely with the rest of the clinic's staff.

The clinic offers physician services, specialist nurse clinics, nursing care, child health care, psychosocial support, podiatry, and laboratory services. We also manage several elderly care facilities.

About You

You are a licensed physiotherapist, occupational therapist, nurse, or hold equivalent education and experience. Experience in primary care and previous work with rehabilitation and care coordination is highly meritorious. We value team players who are familiar with the Cosmic electronic health record system. Proficiency in Swedish (C1 level) in both spoken and written form is required.

You can make professional assessments, initiate and conclude treatments, and structure your work effectively. You are communicative in both speech and writing, and a problem-solver both on your own and in teams. Personal suitability is highly valued.
