Job Description
Your mission is to work coordinatively for patients who are on sick leave and where care needs to be coordinated with other parties such as employers, the Swedish Social Insurance Agency (Försäkringskassan), the Swedish Public Employment Service (Arbetsförmedlingen), or the municipal social services. In your role as Rehabilitation Coordinator, your tasks include providing support and advice to individual patients and conducting assessments of the individual's rehabilitation needs. You will be responsible for and participate in SIP processes/care plans and meetings with employers. Your duties include supporting doctors and collaboratively creating clear rehabilitation plans. A key part of the role is to coordinate and collaborate both internally and externally, all to support the patient and contribute to preventing and shortening sick leaves.
The Team
We are approximately 35 employees at the health center who enjoy working together in an open climate. As a Rehabilitation Coordinator with us, you will work in close collaboration with the other staff at the health center.
The health center features a doctor's office, specialist nurse clinics, a nurse's clinic, child health care, psychosocial competence services, foot care, and a laboratory. We are also responsible for several nursing homes.
About You
You are educated as a physiotherapist, occupational therapist, nurse, or have other education and experience that we assess as equivalent. Experience within primary care and previous work with rehabilitation issues and care coordination is meritorious. We would like you to have experience working in teams and familiarity with our medical record system, Cosmic. Furthermore, you must master the Swedish language in both speech and writing, corresponding to C1 level.

You have good collaborative skills but are also independent in your way of working. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. You are communicative in both speech and writing and are a problem solver both independently and together with others. We place great emphasis on personal suitability.
