Job Description
Your task is to work coordinatively for patients who are on sick leave and where care needs to be coordinated with other parties such as employers, the Swedish Social Insurance Agency (Försäkringskassan), the Swedish Public Employment Service (Arbetsförmedlingen), or the municipal social services. In your role as Rehabilitation Coordinator, your duties include providing support and advice to individual patients and conducting assessments of their rehabilitation needs. You will be responsible for and participate in SIP (Joint Action Plans) and care plans, as well as meetings with employers. Your responsibilities include supporting physicians in creating clear rehabilitation plans. A key part of the role is to coordinate and collaborate both internally and externally, all to support the patient and contribute to preventing and shortening sick leave.
Work Group
We are approximately 35 employees at the health centre who thrive together in an open working atmosphere. As a Rehabilitation Coordinator with us, you will work in close collaboration with the other staff members at the health centre.
The health centre features a doctor's office, specialist nurse clinics, a nurse's clinic, child health services, psychosocial competence, chiropody/podiatry, and a laboratory. We are also responsible for several nursing homes for the elderly.
About You
You are educated as a physiotherapist, occupational therapist, nurse, or have other education and experience that we consider equivalent. Experience within primary care and previous work with rehabilitation issues and care coordination is meritorious. We would like you to have experience working in teams and be familiar with our medical record system, Cosmic. Furthermore, you must have a command of the Swedish language at a C1 level in both speech and writing.

You have good collaboration skills but are also independent in your way of working. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. You are communicative in both speech and writing and are a problem solver, both independently and in collaboration with others. We place great emphasis on personal suitability.
