About Us

At SSIH (Specialized Home Healthcare), we primarily work with palliative patients of all ages and with various diagnoses. Our goal is to provide patients with a good quality of life during difficult life situations and to enable patients to remain at home even when facing serious illness. We work in multidisciplinary teams consisting of nurses, doctors, occupational therapists, physiotherapists, and dietitians. Our patients reside in both Trosa and Gnesta municipalities, but we operate from our premises at Trosa Health Centre.

Your Responsibilities

Working as a social worker (counselor) in SSIH means that, as part of the team, you provide psychosocial support to patients enrolled in SSIH and their relatives. The social worker's role is to complement medical care with psychosocial knowledge from a holistic perspective. This includes home visits and telephone contact with patients and relatives for supportive conversations during grief and life changes. The focus of the conversation can be supportive, crisis-oriented, processing, or existential. The social worker also participates in family meetings, sometimes together with doctors and nurses in SSIH. The social worker's duties may also include guidance on social, economic, and legal matters. Group activities, such as bereavement counseling, also take place.

Your Qualifications

• Licensed healthcare social worker with a degree in social work.
• Experience in palliative care or home-based patient care is preferred.
• We place great emphasis on personal qualities.
• A valid driver's license is required.

Employment Terms

Permanent position at 40%. Start date as agreed.
