You will base your work from Ullared, where you will be part of a mixed team of assistant nurses and nurse colleagues working across the entire inland geographical area of Falkenberg Municipality. In municipal primary care, nurses, specialist nurses, occupational therapists, physiotherapists, pharmacists, assistant nurses, unit coordinators, and unit managers work together. We have a care planning and education team, and together we work to meet the patient's needs for an independent life with good quality of life. Job Responsibilities

As a nurse, you work for good and safe care for our citizens who are registered in municipal primary care, where patients reside in ordinary housing or in one of the municipality's housing facilities. In Falkenberg Municipality, we have somatic care, short-term housing, day activities, and dementia units.

In your work, you go out on acute and planned healthcare assignments. Your work will be varied as it involves performing independent acute, clinical assessments, palliative care, medication management, advanced nursing care, training and supervising care staff, as well as coordinating and planning health and social care interventions in the patient's home and via phone with care staff.

All licensed professionals (nurses/occupational therapists/physiotherapists, pharmacists) and assistant nurses work in close collaboration with the patient in focus. Together we strive to give the patient the best possible care. Over the past year, we have expanded with colleagues to the care planning team, pharmacists, and an education team.

Requirements and Qualifications

We are looking for you who are a licensed nurse, experienced or newly graduated. Further education in district, geriatrics, or psychiatry is meritorious. For the position, you must have a valid B driver's license. Great importance will be attached to personal suitability.

Employment Terms

Employment type: Permanent.
Duration: Permanent, start: Start according to agreement.
Weekly working hours: 38.25h/week (100%) - Day, evening, and night. We currently work on a 10-week schedule. Approximately 7-8 evenings/10 weeks and currently night duty every 5th weekend.
