Assistance Case Worker – Do you want to make a difference for people in Bengtsfors and Mellerud?

We are looking for you who want to become part of our dedicated team at the Assistance Unit in Bäckefors, Bengtsfors Municipality. Here we work close to the individual with the need at the center – always with a focus on security and quality.

About Us

The Assistance Unit is part of the Individual and Family Care and is responsible for the exercise of authority according to the Social Services Act regarding care and support for the inhabitants of Bengtsfors and Mellerud municipalities. Since 2023, we operate from our office at Dalsland Hospital in Bäckefors. We are in an exciting phase of development with new working methods, team structures, and a senior reception. Here you will receive collegial support, reflective team meetings, and a work environment where we help each other. We have a senior case worker who leads and distributes the work through individual check-ins, joint case guidance, and methodological support.

Your Role

As an Assistance Case Worker, you investigate and decide on the right to interventions according to the Social Services Act, such as home help services, special housing (elderly care), or short-term stays. You are also responsible for following up and evaluating whether the help is working as intended. We investigate using the IBIC methodology. You will meet people in different life situations, collaborate with relatives and other professions, and contribute to a secure transition of care. The work requires structure, legal certainty, and good communication – both verbal and written.

The role of Assistance Case Worker includes among other things:

  • Receiving applications according to the Social Services Act (SoL), investigating, and based on the results of your investigation, making a decision on suitable interventions or a decision to reject. The investigation work includes obtaining information from and collaborating with other professions.
  • Follow-up of decisions and approved interventions in collaboration with providers.
  • Conducting planning for discharge from the hospital to ensure a safe and secure transition of care.
  • Identifying needs for coordination and calling for or participating in SIP (Swedish Interagency Programme) meetings for collaboration with healthcare and other units within the municipality.
  • Providing relevant information and guidance within the framework of our service obligation.

We are looking for you who

  • Have a degree in Social Work or equivalent education
  • Are solution-oriented, stress-resistant, and thrive with both independent work and teamwork
  • Have a B driver's license and good computer skills

Meritorious is experience with assistance case work, IBIC, SIP meetings, Lifecare VoO, and Samsa.
