Region Örebro County is developing primary care practices for patients with extensive and complex care needs. The increasing proportion of elderly residents, especially those over 80, creates a need for strengthened continuity, holistic assessments, and coordinated interventions. Nora Health Centre is now looking for a nurse or district nurse with a special interest in geriatrics and care coordination. We offer:

The collaboration with Nora Municipality's health and medical care services for the elderly is very well-functioning. We have interesting and important development projects underway in cooperation with the municipality, for which we have received state grants.

The position involves a combined role consisting of 50% district nurse and 50% care coordinator. The clinical work as a district nurse includes planned clinic visits, telephone advice, home visits, and cases via digital contact channels, as well as associated administration. The role as care coordinator is shared with our current coordinator and involves planning and follow-up of patients' care interventions in close collaboration with the municipality's coordinators.

Requirements: Registered nurse or district nurse. Experience and interest in the patient group. Good ability to collaborate with internal and external actors. B driver's license. Merits: Further education in geriatrics and/or cognitive impairment. Experience in care coordination.

As a person, you are confident in your professional role, have a good attitude towards others, and can work independently and in a team. You are structured and have an understanding of the systems surrounding the patient and their relatives.
