About the Social Services Administration and Home Care

The Social Services Administration is responsible for measures, support, and services to individuals and families in the municipality. Our operations are primarily governed by the Social Services Act, the LSS Act (Act concerning Support and Service for Persons with Certain Functional Impairments), and the Health and Medical Services Act, and are to implement measures regulated by these laws. All operations are characterized by a person-centered approach to meet the individual's needs. The role involves a strong customer focus and a service-oriented attitude.

The Home Care in Motala Municipality consists of occupational therapists, physiotherapists, nurses, nursing assistants, and rehabilitation assistants. We work in teams together with elderly care, LSS activities, and day activities for the benefit of our citizens.

Job Responsibilities

We are currently looking for a nurse for a permanent position. As a nurse in KHSV-Municipal Health and Medical Care, you will work with Health and Medical Services Act (HSL) measures for registered patients in ordinary housing. During weekends, the work also includes performing HSL measures in special housing, such as elderly care homes and dementia care facilities.

Home care can include medical interventions, rehabilitation, and nursing care. Assignments range from limited simple care measures with intervals of several days to healthcare assignments with multiple visits and measures per day for severely ill patients. The assignment implies proximity and continuity with a holistic perspective in patient contact. It is a varied and exciting assignment with good opportunities for personal development and responsibility.

Your main tasks include:

• Making independent assessments and leading and performing health and medical care measures.
• Planning measures and conducting follow-ups.
• Handling medications, infusions, and medical technical equipment.
• Working preventively and proactively in contact with the patient.
• Working person-centered and planning work based on the individual's conditions and needs.
• Responsible for leading and delegating nursing staff in HSL tasks.
• Working in close collaboration with the unit manager, nursing staff in ordinary housing and home help services, and in contact with relatives.
• Serving as an important contact and having close cooperation with primary and specialist care in Region Östergötland.

Requirements and Qualifications

• Licensure: You must be a licensed nurse.
• Driver's License: You must have a Category B driver's license for a car with a manual transmission.
• Language: Good knowledge of the Swedish language, both written and spoken.
• Experience: Experience in the nursing profession is meritorious.

• Salary: Monthly salary.
• Employment Type: Permanent position.
• Duration: Permanent, start date: According to agreement.
• Working Hours: Full-time (38.25 hours/week).
