Regional Coordinator for Center for Rare Diagnoses Central Sweden (CSD) with Clinical Role in Hudiksvall

At our Child and Adolescent Health Center in Hudiksvall, you will have a meaningful assignment where you combine close patient care at our clinic with work within the Center for Rare Diagnoses Central Sweden (CSD), where you support families, drive development, and collaborate regionally. Job Responsibilities
As a regional coordinator within CSD, you will have an independent and stimulating role with responsibility for contributing to a better quality of life for people with rare health conditions in Region Gävleborg. You are part of a regional network of CSD coordinators and have close contact with CSD's secretariat in Uppsala, which leads the work. The assignment involves:
- Guiding healthcare professionals, patients, and relatives in care and societal support systems.
- Mapping needs and creating need plans together with patients and relatives.
- Coordinating contacts between healthcare, schools, social services, the Swedish Social Insurance Agency, LSS, and others.
- Organizing parent meetings, theme days, workshops, and lectures.
- Participating in regional and national development work, including expert teams.
- Contributing to the implementation of care programs, knowledge support, and quality registers.
In the clinical part of the position, you work at the Child and Adolescent Health Center in Hudiksvall with tasks relevant to your profession and the unit's needs. The work is done in close collaboration with the team focusing on the child's and family's needs.
This may include, for example:
- Assessment, counseling, and support for children and families.
- Clinic work within outpatient care.
- Involvement in day care and home care where applicable.
- Patient and relative contacts in cases of complex and long-term care needs.
- Coordination and teamwork with a focus on the best interests of the child according to the UN Convention on the Rights of the Child and NOBAB standards.
For nurses, the tasks may also include medical preparation, treatment, sampling, and telephone counseling.

Qualifications
We are looking for someone who is confident in their professional role and enjoys working independently, taking responsibility, and driving their tasks forward. At the same time, you value close collaboration with others and contribute to a good team climate. You are structured and communicative, with an ease in creating good contacts with colleagues, patients, and relatives. In a operation where conditions can sometimes change quickly, you are flexible and adaptable, meeting different situations with a calm and solution-oriented approach.
For the position, you must:
- Be licensed within healthcare, such as a nurse, physiotherapist, or dietitian, or have other relevant education and experience.
- Have good knowledge of Swedish in speech and writing.
For this position, it is meritorious if you have experience working in pediatric care, schools, Child Health Centers (BVC), or other operations with children and families, and experience working with rare conditions. It is also meritorious if you have experience in coordination or working with patients with complex care needs. It is an advantage if you have an established network within healthcare and community actors. Personal suitability is highly valued.
