Are you a nurse and want to work with coordinating municipal primary care? We are now expanding our team with a coordinator for Home Health Care in HSV Sydost. We warmly welcome your application!

About the position

As a nurse coordinator in home health care, you will have a key role with us. You will primarily work with administrative tasks, alternating with periods serving as a nurse where you perform healthcare interventions at the homes of patients aged 18 and older who need health and medical care at home. The coordinator role involves, together with your coordinating colleagues, taking calls from partners, patients, and relatives, leading and distributing tasks to nursing colleagues, and having overall responsibility for various administrative duties. You will also be responsible for incoming care requests for patient admissions and the associated handling.

The coordinator function was introduced at the workplace in spring 2025, making you important for continuing to develop the role. As a coordinator, you will meet and collaborate with many functions, becoming the "hub" of the network. The workplace includes nurses, occupational therapists, and nursing assistants.

Examples of duties:

  • Receive incoming calls from patients, relatives, and partners
  • Plan and organize care interventions
  • Decide on and follow up on care measures
  • Monitor planned discharges via Lifecare SP
  • Plan for admissions
  • Collaborate with home care administrators
  • Document in various quality registers and business systems
  • Participate in and drive continuous improvement and development of our operations
  • Perform health and medical care interventions at the patient's home

About the workplace

HSV Sydost is one of five home health care areas in Örebro Municipality. Our catchment area consists of urban and rural areas in a varied and pleasant region. We are based at Ladugårdsängen, located above Ängen Health Center. As a nurse with us, you are offered exciting work with patient groups of different ages. Qualifications

As a nurse, you are confident in yourself and have good self-insight. You enjoy having your own areas of responsibility and take responsibility for your tasks in an independent and structured manner. Being responsive and accommodating in your interactions, both with colleagues and clients, is second nature to you. You are clear in your communication and have the ability to ensure that information reaches its destination. Furthermore, you adapt easily to changed circumstances and see possibilities and solutions in what you face. Additionally, you master the Swedish language to a level that allows you to communicate with clients and other staff, as well as document what the operation requires. Since the work involves digital documentation in business systems, good computer skills are required.

We place great emphasis on personal suitability.

Requirements:

  • Registered Nurse (RN)
  • Category B driver's license

Merits:

  • District Nurse
  • Experience in telephone advice
  • Experience working in municipal or regional primary care
  • Experience working in business systems such as Treserva, Lifecare, Pascal, and Senior Alert

Scope and start date

Employment type: Permanent position

Full-time: 100%

Start date: As agreed

Working hours: Weekdays and public holidays falling on weekdays
