About the Role

The dementia nurses are part of Ronneby's rehabilitation unit. On this unit, approximately 10 occupational therapists, 9 physiotherapists, 2 rehabilitation assistants, 2 dementia nurses, and 3 Silvia Sisters work. The unit operates as a team. The dementia nurses and Silvia Sisters have an additional collaboration as a dementia team, whose operations we are currently developing. The dementia team also collaborates closely with the municipality's social secretaries and the team for safe home return.

Responsibilities

Currently, there is 1 dementia nurse per 15,000 citizens, which in Ronneby means there are 2 dementia nurses. We work across regional/municipal boundaries and in a consultative capacity. We work based on regional guidelines for dementia as well as locally designed routines.

As a nurse specializing in dementia care, you work person-centered in ordinary housing but may also have assignments in the municipality's care and nursing homes.

As a nurse with a dementia focus, you work independently but are also an important part of the team surrounding the patient. The transition to "Close Care" (Nära vård) means that more patients are cared for to a greater extent in their own homes from a salutogenic perspective, where we strive to maintain health in the individual. Close Care implies a patient-centered approach with the individual at the center.

As a dementia nurse, you participate in the investigation of cognitive difficulties. You participate in various collaboration groups. You are involved in the testing and prescription of certain specific assistive devices. You have an advisory and educational role for other staff.

The work includes support for relatives through relative groups, relative education, phone calls, and individual conversations. Your daily work involves collaboration with other actors, for example: Social secretaries, unit managers, home care nurses, health centers, and the cognitive clinic.

Working hours are during daytime, Monday to Friday.

Qualifications

You who apply are a registered nurse and preferably with specialist education in dementia care, district nursing, psychiatry, or elderly care. We would like you to have experience as a nurse and to be secure in your professional role, working for a preventive approach with the individual's needs at the center. As a person, you are structured, flexible, have high collaboration skills, and are solution-focused. For us, it is important that you are empathetic and receptive in your mission when meeting people. Great importance is attached to personal suitability.

In the role of a nurse, you document and communicate daily, so you need to be able to express yourself well in Swedish. We also want you to have good IT skills, be familiar with handling medical record systems, and be interested in developing welfare technology.

Since the administration has operations throughout Ronneby Municipality, it is a requirement that you have a B-driver's license; a B license with a manual gearbox is meritorious.

To obtain employment within the care and nursing administration in Ronneby Municipality, an extract from the police criminal record must be presented. The form to be used is called "Work in the home with elderly and persons with disabilities." The normal processing time is approximately two weeks. Remember to order the extract in good time.
