Home Care Service South, Frövi & Fellingsbro

The Home Care Service South is based in Frövi and Fellingsbro, with coordinated planning between the two locations. We perform approximately 500 visits daily to individuals with decisions on assistance or home nursing care. We reach our clients on foot, by bicycle, or by car. Your colleagues are assistant nurses and care assistants. The team on-site at Tallen also includes area-responsible nurses and occupational therapists. The unit is led by a unit manager, a business administrator, and an administrative planner. We collaborate with other units, and introductions to other operations within Care and Social Services will become relevant.

Job Responsibilities

You will work in a rewarding and stimulating work environment, where the focus is on helping individuals with varied tasks based on their needs.

The work involves performing usual care tasks in the individual's home according to implementation plans, performing health and medical care tasks after delegation, potentially serving as a permanent care contact (if you are an assistant nurse), and documentation.

We work based on Lindesberg Municipality's value base, which means the work is shaped with the individual's needs and wishes at the center. The work also involves supporting and motivating the individual to achieve independence and participation in their daily life, as well as having an active and meaningful existence. Documentation, participation in workplace meetings, and collaboration with relatives, district nurses, and occupational therapists are part of the assignment.

We offer you an introduction where you will shadow experienced colleagues to become confident in the role. During the introduction, you will gain knowledge of our value base, which all staff work from. You always have close access to colleagues who provide support when needed.

Qualifications

• Good computer skills.
• Good written, spoken, and reading skills in Swedish.
• You need to be able to work independently and in a team; you are flexible, solution-oriented, and responsible.
• Ability to cycle.
• Willingness and ability to take on delegations as needed by the operation based on the nurse's assessment.

Preferred Qualifications

• Trained assistant nurse with a professional certificate from the National Board of Health and Welfare.
• Category B driver's license.
• Experience from home care services.
• Experience working from a rehabilitative approach.
• Familiarity with the business system Viva.
• Familiarity with the planning tool Intraphone.
