About the position

You meet people at all stages of life and become an important part of a professional team that together creates continuity, quality, and care in healthcare. You have a central role in the operations with responsibility for both telephone advice and reception work via both physical and digital visits. We work in systems such as TakeCare, Lifecare, and Vårdportalen.

There is an opportunity to have your own clinic based on areas of interest, such as heart failure, elderly care, wound care, etc. We also work in teams regarding mental health, which means there are great opportunities to work with this patient group.

Your responsibilities include among other things:

• Wound dressings.
• Annual checks for conditions such as hypertension.
• Advisory conversations.
• Telephone advice.
• Vaccinations.
• Participation in our continuous improvement work.

The position is scheduled on weekdays without holidays. The extent of employment can be discussed. The employment is permanent.

About you

For this position, you need:

• To be educated as a district nurse or registered nurse with at least a couple of years of experience.
• Have very good knowledge of Swedish, both spoken and written.

It is meritorious if you also have:

• Worked at a primary care center previously.
• Knowledge of the medical record system Take Care.

You are flexible and thrive in an operation where the pace can vary and where workdays rarely look the same. You have good collaboration skills and appreciate working in a team, while at the same time being confident in taking own responsibility and working independently.

About us

Hallstvik Primary Care Center is located in the northern part of Norrtälje Municipality and is Region Stockholm's northernmost primary care center. We have approximately 4,800 registered patients and 16 employees. Here you become part of a team consisting of pleasant colleagues with high competence and long experience. We are a stable work group with good atmosphere, strong team spirit, and short decision-making paths, which is the advantage of a smaller primary care center. The distance to the emergency hospital and urgent care in Norrtälje (approximately 40 km) means that we often meet patients with more acute conditions.

Tiohundra operates five primary care centers and a branch primary care center in the area. We collaborate on quality and development issues with both the hospital and other operations within the company.

We offer a wide range of clinics and competencies, including:

• Team for mental health.
• Counselor.
• Child Health Center (BVC) and Maternity Health Center (MVC).
• Several nurse-led specialist clinics, such as diabetes, asthma/COPD, heart failure, hypertension, elderly care, and cognitive impairment clinic.
