We are now looking for specialist nurses with a care coordinator role for our care and nursing homes in Kärna, Diseröd/Romelanda, and Central Kungälv.

Responsibilities

As a care coordinator, your task is to ensure that residents feel they have influence over their daily lives and receive safe and secure care, so that the elderly experience a dignified life and well-being in accordance with the national values of elderly care. We must work with respect for the elderly person's privacy and personal integrity, and design care and support based on individual needs.

The role involves pedagogically supporting and developing nursing staff to become knowledgeable, confident, and independent in their care work. It also means providing staff with tools to work from a person-centered approach and increasing competence in interacting with residents and their relatives.

As a care coordinator, you will spend a large part of your working hours on the units, directly connected to the care work. Your working hours are mainly daytime, Monday to Friday, but evening work, weekend work, and night shifts may also occur. The position is 100% full-time.

The work as a specialist nurse with a care coordinator role includes, among other things:

• Monitoring the initiation of new cases and the closure of existing cases within the units according to the social services process.
• Coordinating efforts between different professions, for example, by participating in team meetings.
• In complex cases or situations, providing practical support in a needs-oriented and systematic approach together with the resident and their designated care contact. From this, the role includes performing care interventions individually to best guide employees. The role also involves conducting follow-ups on these cases.
• Being part of the introduction of new employees.
• Participating in (or leading) operational meetings and reflections at the base unit.
• When necessary, participating in Senior Alert and BPSD assessments and the creation of interaction plans and other action plans.

Qualifications

For this role, we are looking for you who:

• Are educated as a specialist nurse with 200 YH credits.
• Have the ability to supervise individuals and groups.
• Possess pedagogical skills and good teamwork abilities.
• Are well-grounded in the salutogenic approach and the national value base.
• Are well-versed in person-centered care and support.
• Have a good ability to communicate clearly in both speech and writing.
