Hedesunda Färnebo Health Centre is located 3.7 miles south of Gävle. The catchment area covers Hedesunda and large parts of Österfärnebo and Gysinge. The population base in the area is approximately 3,900, with 3,350 actively registered. Hedesunda Färnebo Health Centre is part of the Primary Care Collaboration Area Gävle, which consists of eight publicly run health centres. We provide primary care services including a doctor's clinic, respiratory clinic, home visits, child health services (BVC), district nurse clinic, psychosocial team, occupational therapy, and physiotherapy. We are a small and pleasant health centre with a joy for work and excellent teamwork. We continuously work to maintain a good working environment.

Job Responsibilities

Are you an occupational therapist who wants to work with us to improve our patients' opportunities for activity and health? The position is offered on a 100% basis. The need for stationary care at Hedesunda Färnebo Health Centre is 50%, and the remaining 50% involves working within the team in the Primary Care Collaboration Area Gävle.

As an occupational therapist with us, you will work with among other things:

• Assessment of activity capacity in consultation with the patient and needs for rehabilitation measures.
• Assessments, prescription of aids, fitting, and follow-up according to the prescription process.
• Memory investigations as a complement to the district nurse's and doctor's assessment/investigation.
• Drafting statements in cases regarding housing adaptation, as well as applications for personal assistance.
• Control of aid costs and rental responsibility.
• Patient education and counseling.
• Individual treatment to a high degree in collaboration with other professions.
• Assessments and occupational therapy measures in the patient's home.

Qualifications

We are looking for a flexible and engaged colleague who enjoys being part of the development at the health centre and within primary care. You are responsible and solution-focused with good collaboration skills, as we have interdisciplinary cooperation.

In the role of occupational therapist, you must:

• Be a licensed occupational therapist.
• Hold a valid Category B driver's license.

For this position, it is meritorious if you have experience working in primary care and/or home healthcare.
