We are now looking for an LSS Case Officer for the authority group within the Social and Care Administration, and the target group for the position is individuals of all ages with disabilities, both LSS and SoL. In the unit, we have a shared responsibility to develop the operations according to set goals and work with quality issues where the user is always at the center.
The authority group consists of five case officers within LSS and social psychiatry, nine case officers within SoL elderly care, one senior case officer, and one case officer for housing adaptation. The unit also includes support for relatives, leisure, and culture within elderly care, as well as personal representatives.
The working hours are primarily flexible, but in the transition to a more preventive and accessible operation, there may be deviations in working hours outside of regular working hours.
JOB RESPONSIBILITIES
As a case officer in the disability area, you are responsible for ensuring that individuals with disabilities receive support and interventions according to LSS and the Social Services Act (SoL). The work is based on legally secure case handling, a good approach, and a holistic perspective on the user's life situation.
Your tasks include:
- informing and guiding individuals and relatives about rights, support, and interventions
- describing the application process and ensuring that the user understands their opportunities for participation
- receiving, registering, and processing applications
- conducting needs assessments and investigations according to IBIC (Individual's Needs in Focus)
- collecting information through conversations, home visits, and collaboration with other actors
- assessing and making decisions about interventions in accordance with applicable legislation, guidelines, and case law
- documenting clearly and securely in our operational system Treserva and communicating via Cosmic Link where required.
Collaboration is an important part of the authority's operations. You will work with providers of municipal services, health care, schools, guardians, relatives, and other organizations. You will also participate in coordinated individual plans (SIP) when interventions need to be coordinated between multiple authorities.
QUALIFICATIONS
We are looking for someone with a degree in social work or social and care education. You should have good knowledge and experience in authority operations and the documentation requirements within the field. You have good communication skills in the Swedish language, both spoken and written, and possess good IT skills. It is an advantage if you have good knowledge of the IBIC method, Individual's Needs in Focus, as well as experience in documenting according to the method. Since you will need to drive as part of the job, a B driver's license for manual transmission is required.

It is important that you are flexible and see opportunities as conditions may change. You need to be attentive and have good collaboration skills as you will contribute to an open climate where knowledge is shared and common goals are achieved.

According to the Social Services Act, a police record extract must be presented by individuals before they are hired to work in care homes, supported housing, or protected housing.
