JOB RESPONSIBILITIES
As a Psychiatric Nurse, you will be responsible for the care and planning of the patient's health and medical interventions, within both psychiatry and LSS. The focus is on supporting, assisting, and preserving functions, sometimes in the form of supportive conversations. You will act as a primary care contact and be responsible for coordinating care for the patient internally within the municipality as well as with external healthcare providers. Managing and administering medications according to prescriptions and following up on medication and its effects is also part of the role. The patients you will be responsible for have varying care needs; some require a lot of support while others manage many aspects independently. Many patients have medication overuse, either fully or partially, which occurs in consultation and prescription from the treating physician. Sample collections and various somatic care interventions, such as wound dressings, diabetes care, and palliative care, are also included.
You will guide and delegate to nursing staff and maintain good contact with relatives. You will work both independently and in close collaboration with colleagues in the team. You will use assessment tools in evaluations, register in the databases we are connected to, document, and follow routines.
QUALIFICATIONS
We are looking for someone with a specialist education in psychiatry.
Experience in municipal primary care, outpatient or inpatient psychiatric care is an advantage. You should be able to work independently, structure, and plan your work both for the day and in the future. You need to convey calm and security, take responsibility, and have good prioritization skills. You have a good approach to everyone you meet in your work, and as a person, you enjoy collaborating and see the team as an important part of the work.
The position requires good computer skills and a curiosity for digital working methods. Lerum Municipality is working on digitalization that simplifies working methods and improves conditions for residents. Since communication with patients, their relatives, and colleagues is a significant part of the assignment, it is essential to have good knowledge of the Swedish language in both spoken and written form.
The work requires you to have a driver's license as you will need to travel to our patients located throughout the municipality.
