The forensic psychiatric clinic in Säter is responsible for highly specialized care at the county, regional, and national levels. We mainly care for patients referred for treatment under the Forensic Psychiatric Care Act. We have a total of seventy-eight care places divided into seven departments staffed by experienced nurses and caregivers. The clinic also has specialist doctors and a multidisciplinary treatment team.

Coordinator/Nurse Mon-Fri, Dept 35 Forensic Psychiatry in Säter Job Responsibilities

The coordinator has the following tasks:

• Promote a person-centered working method and increase the participation of patients and relatives
• Participate in rounds and promote active, functioning care processes and patient flows in the unit, according to current guidelines
• Continuously support and remind to communicate with care neighbors regarding admissions, planned discharges, etc.
• Be a partner for fixed care contacts in outpatient care and primary case manager in the municipality
• Educate and guide staff in the unit regarding guidelines for collaboration during discharge, discharge planning, and the establishment of personal health plans
• Call and participate in SIP meetings
• Support, coordinate, and stimulate staff in the unit in nursing work, documentation, and record-keeping
• Consolidate information regarding patients in the department for the staff group

As a coordinator, you work Monday to Friday and serve as an important continuity bearer for the department.

Quality Work:

• Assist the unit manager in revising, developing, and implementing new instructions and routines
• Collaborate with the clinic's coordinators to promote the implementation of management decisions in the operations
• Collaborate with representatives from other healthcare services and municipalities to improve and develop cooperation

Follow-up Work:

• Follow up and monitor that discharge planning is carried out in the journal system
• Follow up and demonstrate how the department works towards and achieves set goals (for example, care plans and personal health plans) in collaboration with the unit manager
• Follow up on how the guidelines for collaboration during discharge are followed and function towards care neighbors

The coordinator works collegially with the regular nurses beyond their tasks, when the regular nurse is not present or as needed, the coordinator performs the nursing tasks necessary for patient-safe care. Delegation is being initiated in the departments so that certain tasks can be delegated to caregivers by the coordinator. For example, distributing medication, taking samples, and giving injections. When a nurse is present, the coordinator does not have medical responsibility.

Requirements:

• Licensed nurse

Meritorious:

• Specialist training in psychiatry
• Experience in psychiatry

We are looking for someone with good collaboration skills who wants to work in a team with the patient as a co-actor. We would like you to be interested in working with care and treatment efforts for patients. Great emphasis will be placed on personal suitability.
