We are now looking for a nurse for the Primary Care Team, Elderly Team at Kungälv Hospital.

About Us

We are the close mobile specialist team providing good care for elderly patients in their homes.

From spring 2026, we will also introduce an “Elderly Team in the Emergency Department,” which will be staffed by nurses from the Primary Care Team, care coordinators, rehabilitation staff, and doctors from geriatrics. You will rotate between the Primary Care Team and the Elderly Team in the emergency department.

It is desirable that part of the position also includes weekend work within inpatient care for stroke/TIA and geriatric orthopedic patients.

Through a person-centered working method, we create conditions for secure patients that lead to a reduced need for inpatient and emergency care, achieved through:

• close contact between the patient and the primary care team
• good continuity and competence within the team
• collaboration and coordinated efforts between the patient's needs, hospitals, primary care, and municipalities
• a pleasant and developing workplace.

Close care is the care that takes place close to the patient, primarily in the patient's home, and the Primary Care Team based at Kungälv Hospital is part of this.

We work to provide security to our most seriously ill elderly by bridging inpatient care with primary care and municipal health and medical care. We work according to the patient's needs with the aim that they can remain in their home environment. We handle acute changes and long-term planning. We also work with a person-centered approach, and the patient is connected to the team based on the need for support and care from the Primary Care Team, ranging from days to months.

Job Responsibilities

The Primary Care Team consists of doctors and nurses. As a nurse in the Primary Care Team, you will have a varied job where you work both as part of a team and independently. You will work according to treatment plans developed within the team. Assignments come from inpatient care, emergency or outpatient clinics, ambulances, or primary care. It is a varied job where you provide treatment, take samples, and follow up at home. You will have a coordinating function around the patient, their relatives, and partners, based on the patient's needs.

You will rotate between the Primary Care Team and the Elderly Team in the emergency department.

Elderly Team

The team targets frail elderly patients seeking emergency care and who live in Ale, Kungälv, Stenungsund, or Tjörn municipalities.

Purpose

The purpose of the team is to develop close care at Kungälv Hospital and offer more patients specialist care at home, thereby achieving a more patient-centered care process.

The purpose is also to:

• optimize the admission process
• reduce the number of emergency department visits for frail elderly patients by having the ambulance contact a doctor in the team before departure to the emergency department, and if necessary, home visits may be offered instead by the regular Primary Care Team
• reduce the frail elderly's need for inpatient care
• prevent non-medical admissions

Qualifications

Licensed nurse with professional experience, preferably specialist, e.g., in elderly care or district nursing.

We place great emphasis on personal suitability.

Conditions

The position is a permanent employment. Rotation position is possible.
