At the Home Care Service in Kungsholmen, you are part of a teamwork to ensure that our elderly residents of Kungsholmen have a functioning everyday life, and we make a difference in people's lives every day.
We operate from two centrally located offices, one on Kronobergsgatan by Fridhemsplan and one on Wivalliusgatan in Marieberg – both close to bus and subway. The unit consists of about 60 permanent employees and is led by two unit managers, together with coordinators, an administrator, and a documentation representative.

Your role
In the role of coordinator, you have a central and cohesive function within the home care service in Kungsholmen. You are responsible for effective short- and long-term scheduling and staffing, focusing on ensuring continuity, quality, and good resource utilization. The work involves extensive contacts both within the organization with, for example, care managers and externally with authorities and health centers.
You will work closely with your coordinator colleagues and two unit managers.
Your responsibilities include:
- Following up on services and orders together with the client's care contact.
- Ensuring that planning is done according to the clients' needs and wishes, as well as according to the Social Services Act.
- Conducting home visits for new assignments and when needs change.
- Guiding and supporting staff in daily work and leading during group meetings and other operational meetings.
- Being responsible for establishing and administering employment contracts.
- Participating in the development of statistics, follow-ups, and operational reports.
Your qualifications and application
You have a high school education in health and care or are a licensed nursing assistant. You also have:
- Current, documented, and several years of experience in an independent coordinating role within home care or other activities in health and care.
- Very good computer skills and experience with operational systems, such as Medvind and Lisa self-service, as well as Schemos, Parasol, Paragå, or equivalent systems.
- Experience working as care staff within home care is an advantage.

You are service-oriented, self-sufficient, and comfortable managing multiple tasks simultaneously and making quick decisions. With good communication skills, you create and maintain trusting relationships and express yourself well in both speech and writing. You are stable and remain calm even in pressured situations.

To work with the City of Stockholm with the elderly and people with disabilities in their homes, you must provide an extract from the criminal record. The extract should not be older than 6 months from the date of issuance.
The Department of Elderly Care is responsible for assistance management and various services that enable the elderly to have their care needs met. Here there are care and nursing homes, service houses, family support, day activities, and home care. The administration applies full-time as the norm, and you will be employed within Kungsholmen's elderly care.
