Sävsjö municipality is a small municipality where there are great opportunities for growth. We are now looking for a nurse for our municipal health and medical care. We work with the patient in focus, and we do this together with rehabilitation and care staff within home care.

We have a total of 25 nurses, of which 4 work nights and one psychiatric nurse. There are also 4 nursing assistants working in the municipal health and medical care.

JOB RESPONSIBILITIES

Your main responsibilities include being responsible for our patients' health and medical care needs at the nursing level. As a licensed nurse, you are responsible for health and medical care interventions, medication management, delegations, and statutory documentation. As a person, you have a good ability to collaborate, communicate, be flexible, and be proactive.

As a nurse with us, you will play a key role in ensuring high-quality care for our patients. Your main responsibilities include: Assessment and Actions: You are responsible for assessing each individual's condition and taking necessary actions based on your assessments. This includes identifying medical needs and implementing appropriate care interventions.

QUALIFICATIONS

We are looking for you who are a licensed nurse, preferably with experience working in home health care. You should have a good demeanor and be professional in your role, be easy to collaborate with, but also have a good ability to work independently.

You have knowledge of the laws that govern the operations. Good knowledge of the Swedish language in both spoken and written form is a requirement, as well as good computer skills.

Medication Management: Part of your responsibility is to manage and administer medication, ensure correct dosing, and document all medication accurately.

Wound Care and Infusion Treatments: You will perform wound care and manage infusion treatments, which require a high level of precision and care.

Preventive Work: You will work with preventive measures to improve and maintain patients' health, including using the national quality register Senior alert to identify risks and develop strategies to prevent care-related problems.

Palliative Care: Part of your assignment is to provide palliative care to support and relieve symptoms in patients with life-threatening illnesses, focusing on improving quality of life.

Teamwork and Collaboration: You will work closely with both internal and external staff. Internally, you will have regular contact with nursing staff, rehabilitation staff, and case managers. Externally, you will collaborate with health centers and other care providers to develop and follow work routines that ensure a coherent care process.
