District Nurse/Specialist Nurse with Coordination Function at Trångsund Home Healthcare

Our clinic is located directly adjacent to Trångsund train station, about 15 minutes from Stockholm City. We have a general practitioner clinic, district nurse clinic, home healthcare, psychosocial team, and our own laboratory. Our 25 employees are responsible for approximately 6500 registered patients. Trångsund is a smaller healthcare center where you will work closely with your colleagues every day.

We can offer a position where you as a district nurse or specialist nurse can truly make a difference and are needed. We have a stable team that we now want to expand with a district nurse or specialist nurse in a coordinating role for our home healthcare.

Job Responsibilities

As a district nurse or specialist nurse with us, you will have a varied job with a focus on the patient. You will work in the Lifecare system for contact with hospitals and other stakeholders around our patients. We hope you appreciate order and structure, where you have the opportunity to work on quality assurance routines for the role of coordinator. We build a strong community within the workgroup, and we value collaboration and communication.

At the healthcare center, we work with programs such as TeleQ, Lifecare, Pascal, and the Take Care journal system.

Qualifications

  • District nurse or specialist nurse
  • Experience in primary care and home healthcare
  • Driving license (B) and driving experience

Employment Type

Permanent position, full-time 100%. A probationary period may apply. Start date by agreement.

  • background checks will be conducted before hiring according to SLSO's overall guidelines for background checks during employment.
  • employees who will have patient contact must undergo a pre-employment examination, according to current directives, which is done through occupational health services before employment.
  • in addition to Swedish professional certification, good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference (CEFR) is required. The same requirements apply to non-licensed personnel with patient contact.
