Psychiatric Outpatient Care, Care and Support Coordination Team, Jönköping

VOSS, which is part of the Psychiatric Outpatient Care, is a care and support coordination team working for people with long-term disabilities caused by mental health issues and in need of coordinated efforts from specialist psychiatry and municipal housing support. We actively strive to find new ways and design good, effective treatment and care. We are now looking for a nurse who wants to be part of developing this operation. The Role of the Nurse

As a nurse in the Care and Support Coordination Team, you will work with coordinated efforts based on the working model; Care and Support Coordination (VOS). The individuals we meet often have complex needs, and it is crucial to see and meet each individual based on their circumstances.

You will work independently as a contact person in close collaboration with housing support and be part of a multidisciplinary team that includes not only nurses and care coordinators but also psychologists, occupational therapists, social workers, and doctors.

The position is a full-time temporary position lasting until 2026-08-31. Start as soon as possible by agreement. Working hours are Monday to Friday, daytime.

Your Future Workplace

The Psychiatric Clinic in Jönköping is one of the largest clinics in the county with both outpatient and inpatient care within specialist psychiatry. The operation has seven outpatient clinics and six wards, as well as a psychiatric emergency department that offers emergency assessments around the clock.

The clinic provides psychiatric specialist care for residents over 18 years old. Parts of the clinic are also a county resource responsible for psychiatric intensive care, addiction care, forensic psychiatry, and emergency services where we handle urgent matters within both the adult psychiatric clinic and the child and adolescent psychiatric clinic.

Within outpatient care, we work with specialist interventions in neuropsychiatry, addiction care, psychosis care, general psychiatry, and psychiatric rehabilitation. We work in teams with assessment, investigation, treatment, and rehabilitation efforts.

The Care and Support Coordination Team (VOSS) is a team that, based on the political goal of Equal Care, targets the most severely ill within the psychiatric outpatient care at Ryhov. Patients are received from the general psychiatric clinic and the Neuropsychiatric clinic (NPT). The aim is to ensure that the patient can maintain contact with the care and support system, ultimately reducing the number and length of hospital stays and increasing the patient's social function and quality of life.

Your Profile

We are looking for a licensed nurse who preferably has further education in psychiatry and experience in psychiatric care. A B driver's license is required for the job as travel is involved.

You need to be flexible and able to plan your efforts independently. You are driven and have a desire to think innovatively and are persistent. Since you will be in contact with many different people, we think it is important that you are a good collaborator and that you have the ability to understand others' perspectives. You work according to a psychoeducational approach, are confident in yourself, and are positive about being trained in new treatment methods. You are engaged with the patients and their relatives.
